JOB DESCRIPTION:

Assess the patient objectively, subjectively in their physical needs, by taking their vital signs, and recording their facts information to the patients progress notes and clinical observation chart.

Plan appropriate nursing intervention for the patient’s wellness short term and long term management according to the case of the patient.

Plan appropriate nursing intervention for patient’s relatives or family in times of patient therapy.

Apply nursing measures and techniques pertinent to medical diagnosis and therapy of the patient.

Monitoring and observing patient’s vital signs frequently to detect any deviations from normal, and to gauge progress of the patient following physicians orders.

DUTIES AND RESPONSIBILITIES:

Provide care to patients after illness, injury or surgery or with acute health conditions or patient who are being treated with pharmaceuticals.

Record, assess and observe patient progress, reactions and symptoms.

Assess needs of patient and develop plan of care base on assessment.

Check vital signs, take the Fasting Hypo count, and Monitor the patient’s condition.

Coordinate referrals and interagency communication.

Manage pain.

Monitor patients in restraint and seclusion and maintain patient safety.

Determine medical or psychiatric emergency need and report immediately to staff Nurse on Duty.

Perform related duties as assigned.

With almost 2 years of experience as a nursing aide in Nursing home from October 28, 2015 up to present here in Singapore, handling of 30 elderly residents who had dementia, BPSD, Alzheimer’s disease, doing the in-charge and assisting the in-house doctor such as geriatrician, and psycho geriatrician, Assisting the Staff nurse on duty during admission, discharge of the resident from the hospital internal transfer to other home, Bathing, Grooming, feeding the residents through NGT and oral, handling of apparatus such as: nebulizer machine, pulse oximeter, glucometer testing device, vital signs machine had been familiarized in times of my practice.
