CAREER OVERVIEW

Humana WAH HCMSS SNP Care Manager (2015-Present)

My role as a Care Manager on the Auto dialer team is to complete telephonically assessments and evaluate Humana members’ needs and requirements to achieve and/or maintain their health. I guide members and their families toward short term and long term goals and facilitate interaction with resources appropriate for their ongoing care and well-being.

• Complete member outreach, engagement, and management activities per program workflow and protocols

• In conjunction with the Case Manager, determine specific short and long-term goals, objectives, and interventions with accompanying timeframes based upon guidelines

• Assists in the monitoring of provider-based discharge planning from all levels of care in accordance with established criteria and makes recommendations where appropriate.

• Maintains an awareness of clinical, social, and financial resources available in the community.

• Complete required documentation.

• Performs duties consistent with all State and Federal regulatory requirements, accreditation organization guidelines, and Humana guidelines

Humana WAH Field Care Manager (2015-2015)

My role as a Field Care Manager consist of being a part of our Clinical Space –engage our members and guide them to develop lifelong wellbeing and health. As a Humana At Home Field Care Manager I make in home visit and use a compliment of instruments that help me identify gaps in care and implement strategies. I work in collaboration with a multi-disciplinary team, employing a variety of strategies, approaches and techniques to enable each member to manage their physical, environmental and psycho-social health issues.

• Evaluate member needs and implement appropriate interventions with Humana Members participants in their place of residence (Home, Assisted Living or Long Term Care Facility)
