Provided individual, group, and co-treatments as indicated per patient's needs and rehab requirements. Coordinated with other rehab staff, nurses and family members, in relation to each patient's progress and treatment goals. Completed patient treatment notes, weekly summaries, discharge summaries and attended weekly staff-meetings, and discharge meetings.

Assisted a middle-aged woman return safely to her home environment after being discharged. Provided guidance with personal safety in the use of a bedside commode chair, functional transfers, bed mobility with a hospital bed and hanging trapeze, safety with shower task; utilizing a wheelchair, full shower chair, and a longhandled shower. Also guided patient in ADL functional mobility using a wheelchair and a walker both in her bedroom and her kitchen. Please see letter of recommendation, dated May 4, 2012.

Developed and implemented various ADL data collection sheets based on time needed, number of cues required, and appropriateness of task follow-through.

Trained staff to assist with data collection and ensure proper documentation. Used ADL programs in conjunction with Behavior Management Programs, which included a point and reward implementation system. Used data collected from Occupational Therapy ADL programs during monthly interdisciplinary staff conferences and teleconferences to show measurability of progress.
