Responsibilities:

Maintains confidentiality of the department.

Quality and Safety Projects: Facilitates project planning and coordination of department led projects that focus on process improvement by providing data from University Health Consortium (UHC), NSQIP and Press-Ganey. Provides expert knowledge of current quality improvement tools and techniques to influence clinical outcomes using methods such as PDSA (Plan-Do-Study-Act), DMAIC (Define- Measure-Analyze-Improve-Control),Value Stream Improvement (VSI), Failure Mode Effect Analysis (FMEA) and Lean Six Sigma.

Quality Initiatives: Facilitates and directs quality initiatives based on core measures, patient safety indicators, patient experience and adverse event reports. Reviews/facilitates review of UHC, Press-Ganey and NSQIP reports at clinical department quality and safety committee meetings. Works with committees to improve patient outcomes related to core measure, PSI and patient satisfaction results of the department. Encourages and facilitates departments to take ownership of reporting and tracking individual patient satisfaction scores at quarterly meetings.

Core measures chart abstraction (for Inpatient Stroke, Outpatient Stroke, Perinatal and Hysterectomy SCIP, Outpatient Pain management). Sends failed core measure cases to appropriate clinical departments for review and educational/process improvement follow-up with nurses and physicians.

Patient Safety Indicators (PSI): Performs daily concurrent review and pre-billing review of 5 PSIs (PSI09, 13, 15, 17, 18, and 19). Reviews, validates and refers cases to appropriate clinical departments for review, confirmation and educational follow-up.

Quality & Safety Committee: Meets with the Q&S committee chair to discuss department quality and patient safety needs and progress. Attends all department Q&S committee meetings, meeting preps and follow-up meetings with due preparation, or when necessary, assigns appropriate designee. Reviews department Q&S Committee minutes for accuracy prior to distribution to committee members. Assists in developing an agenda that is reasonable and appropriate for each meeting. Serves as a back-up to the Q&S Committee Chair for all agenda items. Performs timely follow-up on all action items as needed. Provides all assigned committees with relevant data, reports and information related to quality and safety issues and events. Ensures committee participation and fosters atmosphere of team work and culture od safety among committee members across different disciplines. Encourages resident and nursing participation in all committees. Works in unison with Committee coordinators and communicates as a team to ensure smooth sailing. Presentations Presents the UHC QSMR, HQMR, quarterly core measures reports and Hand Hygiene dashboard at each relevant Q&S committee meeting every quarter and the Annual Quality & Accountability Scorecard. Plans and facilitates relevant educational presentations such as Pressure Ulcer presentation by the Wound Care team, Preoperative Testing Guidelines presentation, Patient Experience presentation by the Performance Excellence team, Clinical Documentation Improvement Program presentation by the CDMP team and PSI/Core Measure presentation by the Q&S team, at relevant Q&S committee meetings.

Case Reviews: Reviews and refers cases, which impact patient quality and safety to the department Q&S Committee for further discussion and review. Reviews and responds to incidents reports, health plan, TJC and patient complaints and provides the first preliminary, objective review. Refers reports and complaints to Q&S Committee chair or designated reviewers. Posts reviews and comments to the RL system and provides trending reports as needed. Tracks reports to assist with identification of quality or patient safety projects/initiatives where needed. Acts as available resource with any external review processes (health plans, TJC, etc.). Coordinates quality and safety case reviews/referrals for review with other clinical departments. Provides support and guidance through sentinel events /adverse events review process by the Patient Safety events committee. Prepares, and facilitates the root cause analysis and assists with actions plans assigned by PSEC.

New Staff Orientation: Assists new staff in their orientation process with PSI review
