Thirty years of nursing experience in Home Health Care, Psychiatric Nursing, Medical/Surgical Nursing, Long Term Care and Telehealth Nursing. Held lead nursing and management positions including; unit manager, primary care nurse case manager, assistant to the Director of Nursing, and clinical supervisor.

Accomplishments:

Assistant Director of Nursing for a new start up Home Health Care agency. Wrote policies and procedures, trained new staff, and did monthly CNA education and evaluations for RN's, LPN's and CNA's.

Monitoring patient’s conditions and evaluating the appropriate level of care being provided by the staff. Organizing and attending Care Planning, IDT, and family meetings. Preparing for, and facilitating, patient discharges. Performing numerous and essential daily tasks.

December 2008 - April 2013: Alere Medical. Multi Disease Primary Care Nurse Case Manager. Case managed more than 350 patients. Case management consisted of education and care for patients with adult diabetes, congestive heart failure, coronary artery disease, COPD and asthma. Methodically reviewing various devices and when symptoms are out of parameters, call and assess the patient’s condition, educate on symptom management, send the Physician a status or alert fax depending on severity of their condition. Responsible for coordinating care with the Physician and/or advising the patient to seek medical attention. Using IMB (Information, Motivation, Behavioral changes) techniques to better provide information, find the patients motivation to make changes in their health and assist in attaining goals. Reviewing goals and outcomes since their last assessment to evaluate progress towards goals, and continued support to reach and/or maintain goals.

June 2007 - December 2008: Complete Home Health Care. Assistant Clinical Supervisor/Clinical Supervisor. Coordinated home care services involving physicians and field staff. Preauthorized home care services in compliance with private insurances, Medicaid, and Medicare. Lead case conferences. Reviewed all OASIS information to ensure accuracy for 485 plan of care.

September 2005 - December 2006: Caring Hearts Home Health Care. Assistant to DON. Coordinated care for home bound clients. Reviewed all OASIS re-certifications, discharges, QA charts, as well as, making home care visits. (Note: leave of absence due to medical procedure).

Supervision of Mental Health Technicians. In-service coordinator for psychiatric unit. Held certification with Crisis Prevention Institute, in-serviced staff in high and low level non-violent/therapeutic take down and seclusion techniques.
