To Whom this may concern:

I mailed in prescription information to you and have talked to two representatives as of

today. I was informed that a check for $529.92 should be here with ten business days. I am

sending you more information on my prescriptions that I seem to not be getting

reimbursed on. The Walgreens Confidential Patient Information Prescription Profile

3/1/2011 through 3/31/2011 shows the information on each prescription I paid cash for

and need to be reimbursed on. The amount I paid totals $721.28. I still am owed $191.36.

I spoke to Robert on Wed. and went through the drug list and according to him I am being

reimbursed on all the drugs except two, Meclizine and Cefprozil. I don’t understand the

reason for this I sent in the info. once, but I am sending this again. If you need to speck

with me again I can be reached at 901-***-**** or 901-***-****. When the check of

$529.92 arrives, I still am due another check totaling $191.36 to cover the total I paid in

cash on all these prescriptions.
