PROFFESSIONAL EXPERIENCE
> Three Years work experience from The Dale View Integrated
Rehabilitation center for Addicts (Thanal), Punalal, Trivandrum as
Mental Health and Substance Abuse Social Worker (1st September 2008 to
13th August 2011).
Responsibilities
. Interview clients
. Evaluate mental or physical condition of clients
. Confer with other professionals
. Plan and coordinate patient treatment
. Make modifications to treatment plans
. Evaluate, monitor, and record client progress

Responsibilities
. Coordinate hospital Care to the patient to cope with diagnosis or
hospitalization.
. Make patient to understand the hospital set up and without ease their
fear to meet the doctor in the particular department.
. Offer Counselling on issues such as drug and alcohol problems,
domestic abuse, child abuse, bereavement, or other issues due to
illness.
. Assess people who are in hospital and maybe in need of social
care services.
. Provide assessments to patients and their carer's and facilitate
a safe and timely discharge through Discharge planning
. Provide a convenient link to community resources and services to the
patient to cope with the medical, financial and emotional issues that
may face during or after hospital stay.
. Conduct home visits to establish patients' coping capacity in their
home environment, linking them with other community resources where
necessary.
. Offer staff counselling for hospital employees who may be affected by
personal, family and workplace problems.
. Conduct counselling and training for healthcare professionals, as well
as corporate clients.
. Conduct awareness classes for the students regarding physical and
mental health.
ACADEMIC FIELD WORK EXPERIENCE
> Three months field work training in Bishop Diehl Rehabilitation Home
for the Blind in Trichy, Tamil Nadu
> Three months field work training in Shree Akhila hospital in Trichy,
Tamil Nadu.
> Three months field work training in Athma- The Mind Centre an
Institute of Psychiatry in Trichy, Tamil Nadu.
> One month block placement training in Mundakappadam Mandirams, an
institution, which provides medical, psychiatric and social services
in Kottayam, Kerala.
> One month block placement training in National Institute of Mental
Health and Neuro Sciences (NIMHANS) in Bangalore, Karnataka
PROFFESSIONAL TRAINING ATTENDED
. Participated in Two Day Sensitization training programmes on
Personality Development for the Alcoholics and Drug Addicts at POC
Ernakulam, Kerala, India, sponsored by National Institute of Social
Defense, MSJE, Government of India on 13, 14 August 2009.
. Undergone training in the area of Addiction Management for 5 days from
31/08/2009 to 04/09/2009, conducted by RRTC South - T.T Ranganathan
Clinical Research Foundation, Chennai, with sponsorship from NISD,
Ministry of Social Justice and Empowerment, Government of India.

Identification and Intervention" conducted by Department of Clinical
