WORK EXPERIENCE
2011 to date- Care Coordinator at Hartwig Care, London
As a coordinator I am responsible for communication with Clients, Clients’ representatives, Care
Workers, Care Managers and Social Workers to ensure the visits to Clients reflect the continuity
and flexibility Hartwig Care promises. I ensure our Care Workers are updated with all necessary
information regarding visits according to agency policies and procedures and I help to improve the
service monitoring as well as the safety of our Clients. I make certain Care Managers are assisted with
communicating with Care Workers regarding daily visits and ensure the agency receives information
and reports from Carers, both verbal and writing, within times set. During the weekends, I hold to On
Call phone, communicating record and monitoring all the activities during out of hours. As a Care
Coordinator I take care of Care Workers’ issues regarding Clients, bookings’ cancellations, Clients’
concerns or changes. I also deal with any concerns and emergency situation during the day and out of
hours.

2007- 2011- Care Worker -Hartwig Care Ltd London
My responsibilities included taking care of elder and disabled people. I provided a personal care,
assistance and support in everyday activities. I have acquired a great experience in handling a wide
variety of challenging situations, related to different sort of disability of people of all ages. I also
attended regular briefings about providing professional care for the clients. Another feature as a Care
Provider was to escort and assist extremely vulnerable and disabled people.
2001 – 2006- Teacher- Special Education Centre for Children and Adults with Learning
Disabilities in Tarnowskie Gory in Poland.
During my 5 years at the Special Education Centre in Tarnowskie Gory in Poland, I worked with
children, teenagers and adults with learning disabilities. I prepared educational materials for all the
class activities and I also adapted teaching methods, according to the students' various needs
and interests. My other duties included good communication with the other Teachers, Educators,
Health Service, Psychologists and parents; all of whom expected extremely high standards of
courtesy, knowledge and experience from me. I dealt with a wide variety of students, with different
degree of disability; both mental and physical. I also participated in The Socrates Commenius
Program where I co-operated with schools and teachers from abroad.
