Supervisor under CLIA ’88 Regulations since 1989, General Supervisor Licensed in State of Nevada
since 1991. Proven proficiency in all areas of laboratory operation. Prior to MT certification, I
performed the duties of an MLT, CLA, and Phlebotomist, with further service in Health Care as a CNA

Assignment to Intermountain Health Care Central Lab in Salt Lake City, UT. Responsible for
isolation and identification of respiratory, prenatal vaginal, and urinary tract microbes, setting
up sensitivities, MIC, and manual methods of identification. Performed all Gram Stains
submitted on culture setups and ER workups, including wet preps and KOH preps. Worked a
split shift of days and evenings, approximately 350 cultures per shift. Extended contract to
work in Rapid Response Lab, Hematology department, performed all CBC’s, Manual
Differentials, CSF, and Body Fluid Cell counts and differentials on night shift, approximately
