School Rehabilitation Counselor
Provide job placement and career counseling to high school students between
to the age of 16 to 22 with mental, physical, and/or development
disabilities. Provide a successful transition from high school to
work/military or from high school to college to the workforce. Duties
include completing intake/ eligibility process, schedule psychological &
vocational evaluations, attend IEP meetings with school administrators,
assist student with applying for college or military, and assist and
counseling to student & family members as well as provide feedback and
information to student and families about deadlines and upcoming events.

Medical Social Worker
Completing in home patient evaluations to assess the patient's need, make
appropriate referrals based on patient's needs to local agencies on behalf
of the patient, and complete follow up contact with patients and family
members after referrals are made to ensure that services are appropriate
for patient and completing discharge summaries about patient's progress to
referring doctor, as well as requesting more visit orders as needed from
the referring doctor. The referrals include local mental health agencies
for mental health assessments to meals on wheels program for seniors (80%
to 85% of patients were seniors). Social worker in Senior Resource Center
duties include intake process to assist seniors with Medicare part D
enrollment, housing issues, food assistance referral, transportation,
community referral for crisis assistance with paying for medications and
rent.
2/ 2005 to 9/2006 Regional HIV/AIDS Consortium, Charlotte, NC
Clinical Services Manager/ SAMHSA Program Director
Supervising the Ryan White Title II and Emerging Communities grants which
covered thirteen counties. The duties included supervising 37 case
managers, completing monthly chart audits and yearly fiscal audits,
completing quarterly reports to submit to the state of North Carolina about
grant outcomes, planning and facilitating monthly meetings for the case
managers and quarterly meetings for Ryan White Service Providers. SAMHSA
Program Director duties included attending all SAMHSA annual meetings and
trainings, completing yearly chart audits of Service providers, planning
and facilitating quarterly meetings for SAMHSA Service Providers,
supervising two staff members full time SAMHSA Coordinators, facilitating
and coordinating training for Service Providers, assist with determination
of allocations for grant funding to Service Providers and recruiting
Service Providers.
12/2002 to 2/2005 Regional HIV/AIDS Consortium, Charlotte, NC

diagnoses of substance abuse and/or mental health issues. Make referrals to
area substance abuse and mental health agencies. Meeting with Counselor and
therapist to assess clients' progress. Complete 6 &12-month follow-ups or
discharges with clients. Attend all annual SAMHSA meetings and training.
