PROFESSIONAL EXPERIENCE
12/12/2011-Current Harbor UCLA research team, Torrance, California
Conduct anthropometric measurements and nutritional assessment on selected renal patients as part of
MADRAD study team of researchers. Part of my job duties are: collecting and calculating and
measuring nutrition related data such as changes in fat and skeleton muscle mass and their ratio. Also
calculating BMI weekly, and documenting 24 hours diet recall from patients.
7/20/2011- Current DaVita HD and Fresenius HD Wilmington/ Ingelwood, California
Educate participants on their therapeutic diet. Design an individual menu for diabetic patients.
Document patient's assessment, reassessments and plan of care and make appropriate nutrition
recommendations and set goals. Work with team of “Bone Management” and as part of team adjust
bone meds. Adjust patients IV Vitamin D monthly with using algorithm tool, and base on their lab
values, and dry weight. Order and re-order labs for participates that have abnormal lab values. Attend in
"Patient Team Care" meetings. Enroll and refer low income patients to agencies such as" meals on
wheel", “Congregate Meals”, Food Banks, etc. as needed and follow up on the status of their
application. Communicate write letter with nursing homes monthly for patients who live in SNFs and
make appropriate nutrition recommendations as needed. Attend in CQI, POC/CIA meetings bi-weekly.

Provide Medical Nutrition Therapy to all the hospital patients. Conducted comprehensive assessments
including TF, TPN, and outpatient clinics. Educated patients on their therapeutic diet. Attend patient’s
team care meetings. Developed a consultation sheet for dietary department in order to improve, and
fasten communication between dietary, and other departments to outreach patient’s nutritional needs.
