Nurse Practitioner Experience
At this time, I am currently working with a variety of clients who are
being seen for psychotropic medication management as well as
establishing primary care. I work closely with other Primary Care
Physicians, PSR workers, Case Managers, and home health staff. All
types of illnesses including UTI, sinus infection, bronchitis, otitis
media, Diabetes, Hypertension, etc. Pain Management was also managed.
All ages of patients treated.
In December 2010, I was hired by EIRMC to assist in transitioning
their outpatient wound care clinic from a Physical Therapist-run
clinic to a Provider-run clinic. I worked closely with a Plastic
Surgeon and a Wound Care Certified Podiatrist in providing care. I
was an intermediary between Clinic Physicians, patients, and referring
physicians. The goal of this clinic transition was to have full-time
provider accessibility, improve patient care, increase volume, and
bring in advanced wound care modalities to better treat patients. As
part of the team, I helped the clinic in becoming successful and a
positive asset to the community.
Frequent communication between me and the patient's PCP and/or
referring physician took place. Using my assessment and diagnosing
skills, individualized treatment and care plans were created. Patients
were assessed holistically to ensure the best care was provided.
I was involved in obtaining DME, wound care supplies, physician
referrals when needed, and scheduling procedures as well as providng
the proper pre-surgical paperwork. When necessary, I admitted patients
to the hospital.
