Utilizes restorative measures in resident care to promote the resident's

highest level of functioning. Takes temperature, pulses and respirations,

blood pressures, records and reports measurement to the nurse. Collects

specimen (urine, stool) as ordered. Ensures the assigned resident's

personal hygiene by administering and/or overseeing bathing, oral care,

hair care and nail care including cutting nails of non diabetic. Provides

positioning) and sign turning sheet. Maintains proper body alignment.

Releases restraints q 2 hrs, performs prescribed activity with residents

and documents on resident's accountability record. Assures that corrective

devices are appropriately used; slings, hand rolls and positioning devices.

Promotes adequate nutritional and fluid intake to assigned residents. Feeds

or assists in feeding of residents as needed. Encourages residents in self-

feeding. Reports food and fluid intake and pertinent problems or

significant changes in the resident's condition to the RN. Weigh residents,

record results - reporting fluctuations to the nurse. Provides fluid as

ordered. Measures and records intake and output. Monitors chair and bed

check alarms. Tests their proper functioning before putting resident in bed

or chair and reports problems to the charge nurse. Provides for the safety

of the residents at all times Participates in assigned residents care plan

meetings. Reads and follows instructions on care plan. Uses proper transfer

techniques. Knowledgeable of wheelchair safety. Assists with nursing care

tasks as assigned. Administers catheter care on each shift for residents

with foley catheters. Transports the residents to and from clinics,

activities, hospitals, etc. Answers residents' call bell promptly. Assist

in admissions, transfers and discharges. Unpacks or assists resident to

unpack personal belongings upon admission. Ensures that all clothing is

labeled. Completes clothing sheets on all admissions, transfers and

discharges. Upon transfer, discharge or expiration; check resident's

belongings against inventory and then packs and sends to the Housekeeping
