Intensive In-Home Team lead (Henderson,NC)

Reaching your goals is seeking a Licensed, Provisionally Licensed or Board Eligible counselor to serve as an Intensive In Home Team Lead in Henderson (Vance County).

1) The IIH Team Leader/LMHP will serve as the team leader in determining overall direction of treatment and providing clinical supervision and guidance to other team members in implementing the plan for treatment.

2) Provide or arrange for other team members to provide crisis management, intensive case management, individual and/or family therapy, substance abuse intervention, parent and/or child skills training, and other rehabilitative supports

3) Provide the bulk of the family therapy and when needed, create behavioral supports for the parents

4)Monitor and manage the child's presenting psychiatric and/or addiction symptoms

5)Responsible for convening the Child and Family Team for person centered planning and is responsible for coordinating the initial assessment and developing the youth's Person Centered Plan (PCP).

6) Consult with identified medical and non-medical providers, engaging community and natural supports, and including their input into the PCP process

KNOWLEDGE, SKILLS & ABILITIES AND TRAINING & EXPERIENCE REQUIREMENTS:

Education and experience to fulfill the requirements of a Licensed Professional or a Provisionally Licensed or board-eligible Qualified Professional actively seeking licensure may serve as the Team Leader conditional upon being fully licensed within 30 months of the date of hire. Considerable knowledge of diagnoses, symptoms of diagnoses, and practice parameters for the population to be served. At least one year documented experience with the population to be served. Knowledge of governmental and private organizations and resources in the community. Knowledge of laws, regulations and policies, which govern the program. Skills in establishing rapport with individuals served and applying techniques of assessing behavioral and psychological needs. Must have basic computer skills. Must exhibit excellent interpersonal skills, strong communication skills, and organizational skills. Ability to maintain effective working relationships with individuals served, agency staff, and other agencies in the community is essential.

Ideal Candidate: Ability to demonstrate knowledge, skills and abilities required by population served. Ability to demonstrate competence by exhibiting core skills including: technical knowledge, cultural awareness; analytical skills; decision-making; interpersonal skills; communication skills; and clinical skills; ability to develop, coordinate, monitor Person Centered Plans and their implementation; basic knowledge of reading, writing and arithmetic; ability to relate to the population served in a professional, supportive, and therapeutic manner; ability to follow oral and written instructions; ability to record basic data and report findings to supervisor; ability to communicate with other staff and members of the community; document through a daily log the time involved in the delivery of services and make summary notes at least weekly that may include the coordination and oversight of the initial and ongoing assessment of activities.
