Men s Soccer Player Prospect Day at Adams State College

Hello everyone,

As head coach for the Adams State men's soccer program, I am writing to invite you to come visit our college. Adams State College

is located in Alamosa, Colorado. Alamosa has numerous outdoor activities to take part in, including skiing, hiking, mountain biking,

kayaking, rock climbing, rafting, hunting, fishing and much more. We are a NCAA Division II school that competes in the Rocky

Mountain Athletic Conference, the most competitive league in the country. We offer a small college atmosphere with a lot of

individual attention. Our college has approximately 3,500 students and offers first class educational and athletic facilities. Our newly

built and remodeled campus offers wireless internet and cable in all living quarters. Adams State College is a great place to earn your

degree while playing soccer.

I have numerous players expressing interest across the country to come and be part of the Grizzly family. Unfortunately, I can t see

every player who has expressed interest. Therefore we will be holding on Saturday August

6th, for boy s that are in high school or graduated from high school. The camp itinerary is as scheduled. We strongly encourage

recruits interested in Adams State College to attend. This is your chance to see what Adams State has to offer and for us to get a good

look at you. If chosen to play for Adams State, realize that our school does offer athletic scholarships.

Please fill out this registration form for the Adams State College Soccer Player Prospect Day.

This day will be held August 6th 2011 on the campus of Adams State College. Training sites will be held on

both grass and turf surfaces. This day will be limited to the first 60 prospects (first come first serve).

Listed below are items that you will need to bring with you.

Indoor soccer shoes

T-shirts

Light jacket/sweat shirt

Soccer ball

Water bottle

Shin guards

Attached you will find a medical release/insurance form. Please fill out registration and waiver below and return

with your payment.

Check in time is 9:00AM Saturday, August 6th, in the lobby of Plachy Hall. Check out will 5:00 PM. If you

WAIVER, RELEASE, ASSUMPTION OF RISK, AND INDEMNIFICATION AGREEMENT

In consideration of the State of Colorado and Adams State College granting me permission to

participate in the above-referenced event, and with the understanding that such participation is

conditioned upon my execution of this waiver and release, for myself, my heirs and assigns, I hereby

acknowledge, recognize and assume the risks involved in the event and any risks inherent in any

other activities connected with the event in which I may voluntarily participate. I expressly assume

the risk of and accept full responsibility for any and all injuries (including death) and accidents which

may occur as a result of my participation in the and release from liability the State of Colorado,

Adams State College and the City of Alamosa, and all of the trustees, officers, directors, agents,

representatives, and employees of the foregoing entities.

I HEREBY WAIVE ANY CLAIM I MAY HAVE AS A RESULT OF MY PARTICIPATION IN

THEABOVE-REFERENCED EVENT. I HEREBY AGREE TO INDEMNIFY, DEFEND

AND HOLD HARMLESS THE STATE OF COLORADO, ADAMS STATE COLLEGE AND

THE CITY OF ALAMOSA AND ALL OF THE TRUSTEES, OFFICERS, DIRECTORS,

AGENTS, REPRESENTATIVES, AND EMPLOYEES OF THE FOREGOING ENTITIES

AGAINST ANY AND ALL CLAIMS, INCLUDING ATTORNEYS' FEES AND COSTS,

WHICH MAY BE BROUGHT AGAINST ANY OF THEM BY ANYONE CLAIMING TO

HAVE BEEN INJURED AS A RESULT OF MY PARTICIPATION IN THE EVENT.

This waiver shall be governed in accordance with the laws of the State of Colorado, and venue for

any action related to this waiver shall be in Alamosa, Colorado. This waiver is intended as the

complete integration of all understandings between the parties. No prior or contemporaneous

addition, deletion, or other amendment hereto shall have any force or affect whatsoever, unless

embodied herein in writing.

THIS IS A RELEASE OF LIABILITY. IF STUDENT IS UNDER EIGHTEEN (18) YEARS OF

AGE, SIGNATURE OF A PARENT OR LEGAL GUARDIAN IS REQUIRED. I HEREBY

CERTIFY THAT I HAVE READ AND FULLY UNDERSTAND THIS WAIVER, RELEASE,

ASSUMPTION OF RISK AND INDEMNIFICATION AGREEMENT.

If you are interested in getting started to become a possible student athlete at Adams State you should do the following

1. Fill out the Online recruit form on our Athletic web page.

2. Register for and complete the NCAA Eligibility Center forms.

3. Have a minimum of a 2.0 GPA in your core classes.

4. Have a minimum composite score of 68 on your ACT, or 820 on your S.A.T verbal/critical reading and math.

5. Apply for Adams State College (Free for soccer players).

6. Apply for all scholarship opportunities by 3/1/2012.

7. Complete FASFA form.

8. Register for housing.

9. Send official transcripts to our admissions office.

10. Send official transcripts and ACT or SAT test score to NCAA Eligibility Center.
