Experience

COMMUNITY BEHAVIORAL HEALTH HOSPITAL - Social Worker - June 2010 to December 2012.

Duties and responsibilities - conduct comprehensive psychosocial assessments and develop individual treatment plans in coordination with the interdisciplinary team. Coordinate and participate in all team meetings to establish the goals of the CBHH patient and develop a treatment plan based on the individual needs of the patient. Record all information regarding interactions with patients in the patient chart. Facilitate Illness Management and Recovery groups with patients covering a variety of recovery topics (i.e. addictions, coping strategies, signs and symptoms of specific mental health diagnosis etc). Coordinate activity regarding legal issues, implement appropriate therapeutic interventions for clients and their families, evaluate effectiveness of treatment interventions, make recommendations for treatment needs following discharge, make appropriate referrals for continued treatment and support following discharge. Facilitate client`s transition to the community or other placement including development of an aftercare plan which thoroughly address all of the patient`s needs after discharge. This position also required keeping in close contact with county case managers and working within the expectations of each individual county.

CLEARWATER SUITES - Assistant Director - June 2007 - March of 2009.

Duties and responsibilities - I performed direct observation of resident care staff including monitoring job performance and conducting performance evaluations, providing staff education, coach and counsel employees, take conduct corrective action as needed and document accordingly. I was responsible for monitoring resident physical and psycho social needs, coordinate with nurses to assure quality care, work closely with resident families, case workers, guardians, physicians, nurses, vendors and other customers of Clearwater Suites. I coordinated and attended care conferences following up with staff to make sure they were aware of changes. I was responsible for planning and participating in facility functions and family socials. I assisted in designing a creative and exciting life enrichment program to meet the individual needs and interests of the resident. Other duties include controlling and maintaining the ordering of necessary medical and cleaning supplies, maintaining proper documentation of resident medical records, generating and maintaining employee schedule for all required shifts, familiar with financial reporting, collation and coding invoices, petty cash management, bank functions, monthly rent functions and payroll management.

BETHANY COMMUNITY - Social Services Director - 2002 to 2007

Duties and responsibilities - I was responsible for assisting residents and families with financial issues regarding nursing home stay, perform case management for 24 sub-acute residents, complete assessments, coordinate admissions and discharges, facilitate a support group for residents and families, facilitate family and resident meetings, assist staff with residents` behavior issues, create discharge plans, perform referral to outside resources, prepare and present in-service training for employees, address concerns of residents and families, oversee the work of two other social service employees within the department, participate in management team meetings, assist in administrative decision making with the management team.

ST. WILLIAMS LIVING CENTER - Social Service Director 1991-2002

Duties and responsibilities - perform case management for 70 residents and 16 assisted living residents, coordinate admission, complete assessments, assist residents and families with financial assistance applications, facilitate family and resident meetings, participate in management meetings, assist staff with residents` behavior issues, create discharge plans, make referrals to outside resources, prepare and present in-service training for employees, address concerns of residents and families.
