Social Work Student Intern

As a student intern at Crossroads Hospice I was fortunate enough to have had the opportunity to work with terminally ill patients. In my capacity as an intern, I performed the following with minimal supervision:

*Assessed and documented patient/family social emotional, financial and psychological needs and resources on the Comprehensive Assessment, within 5 days of admission

*Assessed existing and potential problem areas, needs and resources of individuals, families and significant others

*Participated in Interdisciplinary Group meetings identifying social-psychological problems and interrelatedness with the medical problem

*Involved the patient and/or significant others in determining care goals and objectives

*Planned care for patients, utilizing appropriate situations and involved the patient and /or significant others in determining care goals

*Provided emotional support and counseling services in accordance with the established Plan of Care and in compliance with third party regulations, certified hospice requirements, and standards of practice

*Submitted accurate and complete documentation

*Participated in educational conferences and inservices

*Responsible for maintaining complete confidentiality of patient records, and all other confidential information which may b received during the course of patient care

*Attended QAPI Program activities

*Demonstrated a professional approach with staff, physicians, significant others, patients, staff personnel and others
