SUMMARY OF EXPERIENCE

Current California Licensed RCFE Administrator (large). For over 30-years, my senior level management career in the Healthcare Industry has been divided between full-time employment with local and national organizations, and the full-time or part-time operation of two consulting firms (C. Tate & Associates, and Shanti Ghar Senior Living). After College I started my career in Healthcare by overseeing the operation of seven Industrial Medical Clinics for a major Aerospace company.

I later took on Administrator and Regional Administrator positions in the Skilled Nursing Industry (SNF) and Acute Psychiatric Hospital Industry. I then became an Assistant Administrator, Administrator, and Corporate Operations Analyst in the Acute Hospital Industry. In 1977 when American Medicorp, Inc., was sold to Humana, all AMI Administrators and Corporate Staff were laid off. I then started C. Tate & Associates, a consulting firm serving the Healthcare Industry. In 1995, due to my parents declining health I decided to specialize in Assisted Living/Memory Care, and obtained my RCFE Administrators Certificate. In 2009, I founded Shanti Ghar Senior Living, where I used my knowledge in that industry and my R.E. Brokers license to assist investors with the acquisition and management of Assisted Living Facilities (RCFE). I currently serve on the Board of Directors of Desert Valley Hospital, Victorville, CA 92395

PROFESSIONAL GOAL:

After closing my two consulting firms, my goal is to join a growth oriented organization where my broad range of experience in Administration, Marketing and Development in the Hospital and Assisted Living industries will help ensure that I will become a valued member of their management team.

WORK HISTORY:

Below is a brief overview of my 17-years of qualifying experience for an Executive Director position in the Senior Care industry (Independent, Assisted, and Memory Care).

ENCORE SENIOR LIVING, LLC (2011 - 2011)

Job Title: Executive Director

Accomplishments: Worked with Regional V.P., (Pat Spence) in turning around their Valley Crest Residential Care (Alzheimer's) facility in Apple Valley, CA. Led management team in meeting companies bottom line financial goals for a 64-bed Assisted Living facility providing specialized memory care. Previously Valley Crest had gone through numerous changes in management in a 4- year period. Morale was low, census was down and residents were unhappy, and needed capital improvements had been put off for budgetary reasons. Within 2-months, the staff was working well as a team and providing a high level of quality care. Residents and their families gave many complements to management and

staff. Provided direction to marketing and business departments on ways to increase census and reduce operating costs. Was able to acquire some needed capital equipment for laundry service, by savings on food service costs without reducing quality/quantity of food. Received congratulations from Encore for beating their operational goals for 2011.

SHANTI GHAR SENIOR LIVING, Apple Valley, CA (2009-Present)

Job Title: President/Founder

Accomplishments: Provide advice to physicians and other investors interested in entering the growing market for Independent and Assisted Living facilities. Conducted operational analysis of possible acquisitions to determine what was needed to maximize revenue and improve quality of care. Made recommendations to by or pass. Represented local physician in the acquisition of the Foremost Healthcare Center with 96-beds of AL/ALZ and 67 units of IL, in Hesperia CA for $6.5M (appraised at $12.5M). Worked with owner of Lompoc Residential Care (136-beds) to develop a 28-bed Memory Care unit within existing facility, along with outside recreation area. Represented prospective buyers of a 128-unit Independent Living facility in Las Vegas to convert newer unit into 18-units of Assisted Living, and 18-units of Memory Care. Estimated cost for remodeling was $800K which would raise the value of the property from its $2.8M purchase price to approx. $12M.

DESERT VALLEY HOSPITAL, (153-beds) Victorville, CA (2003 to Present)

Job Title: Member, Board of Directors

Accomplishments: Work with Chairman (Prem Reddy, M.D.) and CEO (Margaret Peterson, PhD) to development and approve operating policies and procedures, represent hospital at various community and government functions, approve actions of medical staff and medical executive committee, participate in annual review of CEO. Assist CEO in recruitment of new members of our medical staff, key nursing staff management and other department heads. Acquired old bank building and converted to Senior Wellness Center. Serve as Chairman of the Desert Valley Charitable Foundation and review and approve applications for college scholarships for students studying health careers. From 2003 to 2006, served as Director of Development and Marketing for both Desert Valley Hospital and Desert Valley Medical Group. As a consultant to the COB, worked on plans for development of a 99-bed IL/AL and a 99-bed SNF on the Hospital Campus with Doug Pancake, AIA.

SILVERADO SENIOR LIVING, San Juan Capistrano, CA (1999-2003)

Job Title: Corporate Director of Development - RCFE Administrator

Accomplishments: Worked with President/CEO (Loren Shook) to develop 10-Alzheimer's Care facilities in California (5), Texas (4) and Utah (1) in a 3-year period. Negotiated site (land or facility) acquisition, and coordinated work with Architects and Contractors. Gained approval from Planning Commissions, City Councils and Department of Social Services. Recovered $3.25-million of the $10-million acquisition cost for a 126-bed facility by subdividing surplus land and then selling off a 20,000/sq.ft. MOB and a 2.5/acre vacant site. Used my RCFE Administrators Certificate in CA to facilitate licensing and opening of facilities. Extensive travel between Corporate Office and job sites, and market research for potential new development/acquisitions.

FOREMOST HEALTHCARE CENTER, Hesperia, CA (1997 to 1999)

Job Title: Vice President Development/Administrator

Accomplishments: Worked with the President (Leonard Crites) on the development, licensing and opening of a 96-bed RCFE and 67-senior housing units on a 16-acre campus. Master planned as a CCRC. Opened on time and under budget with pre-opening reservations 30% higher than projected. Second phase will be the development of a 120-bed SNF/Rehab Center, and another 115 Assisted Living units.

EMERITUS, INC., Phoenix, AZ (1997-1998)

Job Title: Executive Director

Accomplishments: Originally hired to gain experience with Emeritus, prior to taking over a new region they were getting in So. California. After 3-days of a 6-week orientation, the Regional Administrator (Jade Carter) asked me to go to Phoenix to take over a problem facility they had just acquired, and terminated the previous management team. Recruited and trained a new management team, restored resident satisfaction, and grew census from the 60% to 90% range within 7-months. Uncovered embezzlement by Maintenance Director who I terminated, and then did his job along with my job for 3-weeks until a suitable replacement was found. Also worked with the State Director of Senior Housing to revamp AZ regulations to be more like Title 22 in CA.

C. TATE & ASSOCIATES, Apple Valley, CA (1997 to Present)

Job Title: Owner/Principal Consultant

Accomplishments: Consult with private, public and government agencies on a wide range of healthcare related projects. IL/AL projects included : Koelsch Family Senior Living - market research and site acquisition for Sterling Inn (200-bed IL/AL) - Victorville, CA; Desert Valley Hospital - directed planning for development of a 99-bed RCFE and a 99-bed SNF on campus of DVH - worked with Doug Pancake, AIA on design of both projects;

HEALTHCARE CORP., INC. Newport Beach, CA (611-beds) (1968 to 1972)

Job Title: Regional Administrator

Accomplishments: Serve as Administrator of a 136-bed SNF in Lynwood, and oversaw the operations of three 99-bed SNF's and the land acquisition and development of Lyncrest Villa 178-bed Congregate Care Facility. Hired and trained new Administrators and key support Staff. Increased occupancy in all four SNF/s from 75% to 90% range within 7-months. Lyncrest Villa reached 90% occupancy within 1-year of opening. (now licensed as an RCFE)
