Admissions Duties
Handle all aspects intake for new admissions and re-admissions to the facility
Handle all aspects of communication with the family from the moment of inquiry about the facility,
insurance advisement, admission paperwork and d/c planning

Maintain communication with all department heads concerning new admissions and re-admission
including updating them on varies aspects of their health, diet and personal needs
Maintain monthly reports on the number of inquires for admission to the facility including the number
of inquiries denied, lost inquires and admissions Give tours of the facilitySocial Services
Advised families of patients on all different aspects including but not limited to Nursing Home
admissions and dealing with Nursing Home placement of a loved one Handle all aspects of discharge planning including connecting the patient with the correct Home Health
agency and ensuring they receive the proper equipment needed upon discharge from the facility Handle all aspects intake for new admissions and re-admissions to the facility
Handle all facility complaints for all departments including working with the depart head involved to
find a solution for the complaint at hand
Shop for all residents for personal items not provided by the Nursing Home
Conduct weekly care plan meeting along with other department head with resident and family
members to insure continuity of care within the facility including weekly charting on various aspects of
the residents social environment to certain aspects of their health which may affect their social
environment
Marketing /Community Liaison
Maintain relationships with area Hospitals, Physicians, Assisted Living Facilities, Home Health
Companies and Hospice Companies including providing them with accurate information for potential
referrals for our facilities
Develop new relationships with area Hospitals, Physicians, Assisted Living Facilities, Home Health
Companies and Hospice Companies providing them with accurate information for potential referrals
for our facilities by conducting in-services (proving breakfast or lunch) on the services the facilities
provide
Attend community events in order to be further educated on additional services available within the
community which may aid our client base as well as establish a new client base
Attend networking meeting with organizations such as N.O.C.C., N.O.A.H.C.C., and Westbank Social
Networking Organization
Involvement with the Jefferson Council on Aging providing education on Nursing Home Placement as
well as other services that may be helpful to Senior Citizens in the community
Offer goods and services to facilities such as conducting an activity or providing refreshments to a
neighboring facility (senior citizens centers, assisted living communities)
