Developed and submitted the treatment proposal for C.O.M.B.A.T. Funding, and supervised two (2) neighborhood mobilizers. In reference to direct client services, I case managed clients requiring in/out-patient treatment for substance exposure, including community referrals and follow-up. I conducted interviews to assess needs and determine services to be provided; thus supervising the distribution of financial assistance to individuals demonstrating crisis/emergency need.

Senior Case Manager July 1990 to September 1992
Responsible for the coordination and supervision of the Primary Care Integration Program. The 3-year federally funded demonstration grant totaled $400,000.00, and was setup to provide health care and support services to substance abusers and family members in order to decrease the spread of HIV infection. I organized the recruitment, screening, interviewing, and selection of the four (4) key program positions: family counselor, case manager, social service coordinator, and clerk. I composed the program job descriptions, in-house and community referral procedures, developed forms, convened weekly staff meetings, wrote and submitted proposals for the development of community support groups, and organized in-service training programs. I supervised the fiscal and purchasing aspects of the grant program, and coordinated medical treatment, support services with other health care providers. I stocked and updated the health center's Resource Library, consisting of literature, videos, flip charts, and other health education materials. I collected and summarized statistical data and other program information by means of a user profile and monthly reports.

I was responsible for the coordinating the Health Care for the Homeless Program. As a sub-contractor for Swope Parkway Health Center, I interviewed prospective clients and made referrals for medical treatment and support services. I provided social service coordination, case management and outreach. Clientele was a variety of disadvantaged/displaced individuals, families, substance abusers, domestic violence, HIV-challenged, etc...
