Director of Hospice Social Services: Performs Psychosocial Assessments, interfaces with other community agencies and makes appropriate referrals. Counseled patients and families (of all ages) on loss and grief issues. Admission/referral/intake visits. Responsible for coordination of the Plan of Care and follow-through of essential social services. A proactive team member on the Interdisciplinary Team. Short-term counseling on disease progression and loss/grief issues. Counseled many people who were dealing with a mental or other illness besides the impending death of their loved one.

Director of Bereavement: Coordinates and implements the bereavement plan of care utilizing home visits, phone calls and mailings. Conducts grief support groups as needed. Counsels on loss and grief issues within hospice and the community.

Director of Hospice Volunteers: Supervises volunteer spiritual & volunteer supportive staff. Interviews, recruits, and trains new volunteers. Leads, supervises, motivates and enhances volunteer morale which obtains low volunteer turnover. Insures compliance with policies and procedures and mandated state and federal reports and record keeping. Proficient in developing, implementing and presentation of effective training programs.

Director of Public Health Social Services: Phone and in-person crisis counseling to persons of all ages and diversities on available mental and physical health issues with referrals to local,state,federal and independent resources.

Director of Home Health Social Services: Psychosocial Assessments and appropriate community referrals.
