07/2007-09/2002 Registered/Public Health Nurse with Adult: Responsibility included;
w r it ing ini tial nursing assessment, reassessment and quarterly r/t
medical, health, and rehabili tation concerns of medically and mentally
challenged adults. Communicate with other health care professionals
(primary medical physician, Psychologist, Psychiat rist, Pharmacist,
Social Worker, Occupational Therapist, Physical Therapist, Dietitian,
P rogram Director, and Activity Coordinator) to develop care plans,
i mplement and monitor services. Administered routine/prn
medications to 10 to 25 Participants, GT feedings, monitored Blood
G lucose on a daily basis as prescribed by medical physicians and
psychiatr ists. I supervised the implementation work assignments for

I completed the BSN/PHN at the University of Phoenix, Gardena,
California (UOP/Gardena site). I worked as a RN w/Adult Day Health
Care Senior Participants under Title 22 Regulation, affiliated with the
Department of Aging, Health Department/ Medical and the Regional
centers. My job was to wri te TARS for Medical billing using ICD9
coding to the Health Department for Medical approval and payment.
