Physical Therapist
Van Duyn is a 513 bed Onondaga county-owned skilled nursing facility offering a range of
inpatient programs tailored to meet an individual’s unique needs. These services include both
short term rehabilitative care for people recovering from an illness or surgery and long term care
for people with more complex health conditions who need ongoing support. The facility is both
Medicare and Medicaid approved.
Role and Responsibilities:
• Assess and evaluate residents on a referral request.
• Design and implement goals and plan of care.
• Use exercise, modalities, and functional activities, to ease patient pain and help bring
them to their prior level of function or maintain their current level of function.
• Carefully document PT notes using Sigmacare software
• Discharge planning based on the level of function and benefit of skilled PT therapy to the
resident.
• Average Daily Caseload: 7-9 residents/day

rehabilitation, Palliative care or Long term care. The facility is both Medicare and Medicaid
approved.
Role and Responsibilities:
• Assess and evaluate residents on a referral request.
• Design and implement goals and plan of care.
• Use exercise, modalities, and functional activities, to ease patient pain and help bring

• Delegate work and supervise PTAs.
• Evaluate safety in car transfers and home environment.
