Perform case management and social work for transplant patients with a case
load of up to 20 inpatients and 100+ outpatients. Assist patients with
navigating health care system and managing referrals. Team with nurses,
doctors, transplant coordinators, dietary, and respiratory services, among
others. Participant in full hospital on-call monthly rotation involving
cases of child abuse, domestic violence/sexual violence, crisis/trauma
management, and grief support. Facilitator of group (support/education) for
weekly pre/post transplant support. Perform discharge planning and
implementation. Utilize EPIC Medical Recording System.
