6/1/2005 to present: Nurse Practitioner, Chesapeake Neurology Associates, 55 Stoakley Rd,
Prince Frederick, MD 20678. Assess, diagnose, and treat neurological and psychological
conditions. Perform initial and follow-up visits. Prescribe medications, testing and therapy.
Administered trigger point injections and occipital nerve blocks. Interpret and report
labwork. Return patient phone calls, renew prescriptions, fill out required insurance and
disability forms.
6/1/2004 to 5/31/2005: Nurse Practitioner, The Neurology Clinic, 11350 Pembrooke Square,
Suite 312, Waldorf, MD 20603. Assessed and treated patients with neurological conditions.
Performed follow-up visits, although did do initial and first follow-up visits on occasion.
Conducted nerve conduction testing and balance training. Oversaw technicians doing ENG
and EEG tests. Returned patient phone calls and renewed prescriptions.
11/2002 to 4/2004: Cared for my ailing father who had Lewey Body Dementia. Returned to
full time work when he passed away.
9/1998 to 6/2002: Attended Bowie State University for my Post-Master’s Certificate as a
Family Nurse Practitioner. While in school, I did training with Dr Raja Hawitt (Pediatrics), Dr
M Yazdani (Internal Medicine), Women’s Health Clinic at Calvert Memorial Hospital (GYN)
and Genesis Spa Creek (Geriatrics).
9/1998 to 6/2004: RN, Choice Professional Resources, LaPlata, MD 20646. Worked as a
temporary staff RN in area nursing homes, often in a Charge Nurse capacity. Assessed
patients, handled medical emergencies, contacted physicians regarding changes in patient
status, administered medications and performed treatments. Worked in facilities throughout
Southern Maryland.
1/1997 to 8/1998: Clinical Liaison, Chesapeake Potomac Home Health Agency, Hughesville,
MD. Worked with hospital discharge staff and private physicians in ordering Medicare-
certified Home Health Care for patients being discharged home from acute care.
2/1993 to 1/1997: Home Health Nurse, HomeCall, Prince Frederick, MD 20678. Assessed and
treated homebound patients newly discharged from the hospital. Started peripheral and
Midline IVs and ran IV antibiotics, steroids and hydration in the home. Set up and performed
home peritoneal dialysis. Performed wound and various catheter, drain and tube care.
Instructed patients in their disease, care, medication and treatment regimens.
6/1989 to 11/1991: Branch Manager, Nursing Enterprises Home Health Agency, Waldorf, MD.
Oversaw the day to day operations of a busy Medicare-certified Home Health Agency
providing services to the Tri-County area of Southern Maryland. Performed all personnel
activities.
9/1979 to 6/1989: RN. Worked as a staff RN in the following: National Institutes of Health
(National Cancer Institute), Veterans Administration Medical Center and Washington
Hospital Center.
