Responsible for a case-load of 65-70 clients. Completed initial psychosocial assessments,
MMSE’s, and GDS’ as needed. Assisted these clients with housing issues as needed. Responsible
for maintaining accurate records in clients' charts. Active member of the interdisciplinary team.
Provided the interdisciplinary team members with pertinent, up to date information regarding
clients whether that communication was through morning meetings, email or the clients’
electronic medical record. Provided caregivers with resources in the community to assist them
with dealing with caregiver stress. Also made referrals to APS as needed if there were concerns
in regard to client care in the home. Multi-tasked several duties daily, including phone
communication, meeting with clients and family, and attend meetings. A member of TLC’s End
of Life Committee which is equivalent to a Hospice program in the community.

Case-Manager 9/99-11/00
Responsible for a case load of 80-90 clients that were elderly, blind, or disabled. Some clients
were as young as a year old. Also had clients with traumatic brain injuries as their main
diagnosis. Assisted these clients with receiving services through the HCBS/HCA programs.
Completed six month visits in clients’ homes to verify services and to review plan of care with
client and family. Assisted clients with ordering DME equipment, arranging home modifications,
and services outside their home, i.e. day care or transportation. Liaison for these clients to assure
they were receiving the services they needed in the community.
Cottonwood Care Center

Social Services Director on the non-secured unit of a non-skilled nursing facility that specialized
in psychiatric disorders and behaviors for older adults. Assisted with marketing and admissions.
Many admission assessments were conducted at Denver Cares and Ft. Logan. Worked with
residents on discharging to less restrictive environments that would provide adequate services for
their mental health needs. Responsible for developing a care plan for each resident. Responsible
for documentation in medical charts. Scheduled care conferences and invited family and
residents to these conferences. Liaisons for residents to make sure their needs were being met
within the facility and with any outside services they may have been receiving.
Internship
Eagles Club 8/93-5/94
Adult Day Program for men with early on-stage Alzheimer’s disease.
As the Senior Intern I was responsible for running this day program under the supervision of the
Day Program Coordinator. Was responsible for running groups with the men, making sure the
attendees received their medications, documented the men’s intake at the lunch meal and snack
times. Also responsible for assigning duties to two student volunteers that assisted the program
for three hours a day. Reported to the Day Program Coordinator at the end of the day to inform
her of the day’s activities and any concerns. Also assisted DPC with grant writing as needed.
