Objective
The objective for me is to continue to develop knowledge and skills for
effective communication. Working in this field will build upon personal and
professional communication experiences. A position like this will help me
further develop communication skills necessary for the development of
professional competence and values; critical thinking and problem solving;
information utilization; and collaboration. From my time in the military I
have experience as an effective communicator. This job will be a learning

Veterans Case Manager May 2009- Present
- Develop an effective case plan for clients based on individual needs.
- Research database and files for background information on returning
clients.
- Conduct an intake discussion with client, obtaining as much relevant
information as possible.
- Assess client needs for basis of developing individual case plan.
- Review case plan with client, providing resource information,
referrals, and direction.
- Set expectations with client of agency guidelines and compliance with
case plan.
- Support client in achieving case plan goals.
- Conduct follow-up discussions and provide walk-in assistance as
needed.
- Review progress, monitor notes in system from other departments, and
obtain additional information to support client in adhering to the
case plan.

Knowledge, Skills and Abilities:

At my current place of employment my job deals with a lot information
gathering. Research database and files for background information on
returning clients. Conduct an intake discussion with client, obtaining as
much relevant information as possible. After receiving the information from
the client's reasons for being homeless it is my job to help the client
come up with a plan based off the information provided to me.
2. Ability to produce effective written communication products.
Attending the University of Phoenix for 4 years really provided me with an
opportunity to learn about effective communication. Also at my current
place of employment I work to help the homeless as a caseworker for
homeless veterans. We meet one on one and discuss options for ending their
homelessness. In these one on one meetings communication has to be
effective so that the client is clear on the services offered.
3. Skill in effective collaboration and oral communication.
Collaboration and Oral Communication are very important between me and my
clients and other providers. Between me and my clients I have to
collaborate with them to be able to help them see a brighter future if they
are willing to address the issues that have led them to becoming homeless.
