I am the only Pediatric Nurse Practitioner providing quality evidence-based care and patient/family
satisfaction in a private primary care practice located in the vast rural southern tier of New York State.
Dr. Parikh has been one of the very few pediatricians to serve the families of Dunkirk-Fredonia for
over thirty years with a wide range of patient demographics from ethnicity to socioeconomics. I will
continue to gain skills in routine and problem focused history and assessment, health care
management and decision making, treatments and health teaching. I will also focus on community
resources and practical interventions as the rural area with lack of readily available services will pose
new challenges for my patient care. Strong cultural competency will prove to be imperative as I will
be working in an entirely new setting with many different ethnicities.

As a bedside nurse in the critical care setting, I provide high quality care for my patients including
administering medications, drawing labs, changing dressings, caring for central and arterial lines,
managing extra ventricular drains and artificial airways, monitoring and reporting changes and
collaborating with the health care team as well as tending to the emotional needs of the patient and
family. I care for medical, surgical and trauma patients. I serve as charge nurse and preceptor. I
have also cross-trained in the Emergency Department at Women and Children’s Hospital of Buffalo
and take on extra hours weekly where I function as a staff nurse.

Pediatric Cardiovascular Intensive Care Unit, Congenital Heart Institute
Advancing my critical care skills in a specialized unit, I worked exclusively with pediatric patients with
cardiovascular disorders. I provided high quality care for patients pre and post operatively through
their entire course of hospitalization. Each patient presented with a complex anatomy of the heart
that is managed surgically and through cardiac catheterization. As a bedside nurse, I utilized the
skills acquired from experience in a Pediatric Intensive Care Unit but with focus on the unique needs
of a pediatric cardiac patient. I monitor vital signs, ECG and physical assessment thoroughly, perform
point-of-care testing for ABG’s, lactates and electrolytes for fast and acute management, maintain and
monitor patency of drains and chest tubes, employ complete electronic charting and continue to be an
active member of the health care team. I also underwent extensive pediatric cardiac educational
training, basic ECG and 12-Lead ECG totaling 140 hours.

C linical Internships
Hodge Pediatric Clinic: September, 2009 to May, 2010; 300 clinical hours
My primary care training at Hodge Pediatric Clinic served an urban or inner city population’s well and
sick child needs. It was a fast paced teaching environment where I worked one-on-one with my
preceptor, Pat Daigler, PNP. I took an active role as a student in managing the care of patients and
their families, newborn to age 19 years. My main focus was the well child visit. I learned and
provided the management of chronic conditions such as obesity, allergies, asthma, eczema and dry
skin. I also learned in great detail the development of a child and provided thorough and appropriate
anticipatory guidance. I provided much education to parents and patients about their health and
wellness, safety and development, an area where I found myself taking much pride.
Women and Children’s Hospital of Buffalo Emergency Department : March, 2010 to April, 2010; 50
clinical hours
With the guidance of my preceptor, Patti Schwarzkopf-Cornavaca, PNP, I learned and took on the role
of a health care provider in an Emergency Department setting. This is the only Pediatric Emergency
Department in all of Western New York, serving largely an urban or inner city population as well as
suburban and rural areas as far as the Pennsylvania border. I took detailed histories of presenting
complaint and performed physical exams. I made medical decisions as far as diagnosis,
interventions and prescribing medications. I saw a wide range of cases from respiratory illness, viral
syndrome, rashes of varying degree, acute otitis media and bacterial pharyngitis to chest pain or
sports injuries. I also performed minor procedures including removing foreign bodies from superficial
derma or nares, splinting fractures, lancing abscesses, and realigning nursemaid’s elbow.
Pediatric Otolaryngology Clinic at Women and Children’s Hospital: March, 2010 to April, 2010; 25
clinical hours
Following Kelly Smith, PNP, I learned the management and intervention of many pediatric chronic and
acute ear, nose and throat conditions. I had in depth teaching and first hand observation of important
diagnostic testing such as comprehensive audiologic examinations, tympanometry,
nasopharyngoscopy, sleep studies and swallow studies. I observed ENT procedures in the operating
room including tonsillectomy, adenoidectomy, myringotomy and tympanostomy tube placement,
lingual frenectomy and repair of choanal atresia. On a practical level, I feel confident in the care of
allergies, upper respiratory infections, middle ear infusions and infections. As some of the most
common childhood illnesses, treating these problems efficiently has become an important part of my
practice.

Currently
After completing my Master’s degree in Child Health Nursing, I am anxiously preparing for
certification as a Pediatric Nurse Practitioner through the American Nurses Association and the
American Nurses Credentialing Center planned for September of this year. I am working as a
pediatric nurse practitioner at a primary care office in Dunkirk, New York with Dr. Rajiv C. Parikh. I am
gaining excellent experience in this longstanding practice located in a very rural setting. I also
maintain my part time Registered Nursing position at Women and Children’s Hospital of Buffalo in the
Pediatric Intensive Care Unit, Emergency Department and Neonatal Intensive Care Unit. I continue
to precept new ICU nurses, head the PICU Morale Committee and have recently taken up monthly
“mini” research in-services presented to my coworkers. I am in the process of planning permanent
relocation to the Central Florida area where I plan to obtain employment as a nurse practitioner in a
primary care clinic. As I have recently obtained Critical Care Registered Nurse certification through
the American Association of Critical Care Nurses, I would also like to work per diem or part time in the
pediatric cardiovascular, medical-surgical intensive care unit or emergency department at Arnold
Palmer Hospital. I hope to maintain my skills in both specialtiess and continue my dual focus in
health care.
