Authorize services to meet consumer's individual needs. Conduct
interviews with consumers' to assess their potential eligibility for
state health and human services programs. Assess and reassess
consumers' eligibility and service needs. I monitor provider
services with consumers through phone calls, site visits, and
documentation. Serve as a consumer advocate and agency
representative in locating available resources. Conduct referrals to
community resources when appropriate. Document case records using
automated equipment to form a case record for each consumer.

Knowledge of Gerontology, the field of disability, the dynamics of
the aging process, and the special problems of people of aged and

My professional experience consists of interviewing, counseling and in
gathering of information for programs which require applying complex
policies to determine eligibility. I interpret policies to the public,

Completed my rehabilitation practicum in which I recorded 320 hours to meet
the requirement for my core curriculum at U.T.P.A. upon graduation. I
evaluated consumers to distinguish if they meet the criteria for a home
care provider; assisted people with disabilities who have a medical need
and or a functional impairment to determine eligibility for primary health
care services; monitored clients status every three months through home
visits or via telephone to adjust their treatment plans in order for them
to live more independently; handled and review case files with registered
nurses and physicians; conducted progress notes and evaluated each clients
essential needs.
