. Lead improvements in QIQM by initiating weekly care

management meetings to review status of each resident and

ability to impact status prior to upcoming ARDs for the

MDS. 5 Star QIQMs at 100 points.

. Nursing Overtime reduced from 180 hours per pay period to

less than 40 hours per pay day.

. Assisted Regional Team initiatives to accomplish goals at

the facility level.
