I was employed with Shaw IND. from **/**/**** <> *1/28/2009.

I operated a Stretch Wrap Machine and Warehouse Operator taking supplies into the

Departments.

I have an Industrial Vehicle Permit which I can operate the following Lifts:

Elec. Riding Lift-Stand

Elec. Riding Lift-Sit

Elec. Walkie

Elec. Tow Tractor

Fuel Powered Lift

Elec. Cart

Elec. Reach Lift

Battery Changer / Charge;

I also worked in Process Control and printed labels to go on the Products, also I was a

Inspecter /Packer.

I was employed with Stauffer Chemical Company from 1978<>1981.

I was the head print operator on the Print Machine, that printed vinyl wallcovering.

I was employed with LaFrance Mill from 1975<>1978.

My job there consists of working in the computer room in the dye house getting formulas

for chemicals and dyes for cloth. Also run key punch for loading formulas into computer,

organized filing cabinets. And also figuring up the amount of chemicals and dyes to go on

cloth.
