compilation of data reports

Health Educator-Call Center
Provide education via telephone contact to participants with specific
medical conditions (Diabetes, Heart Failure, CAD, Asthma) utilizing
teaching modules set up by CVS/Caremark. Responsible to produce phone time
of 85% of shift with a minimum contact/completed calls for 12 participants
per shift

Assistant OIC Intermediate Care Ward
Assistant Officer in charge of 20 bed Intermediate Care Ward in a mobile 26
bed forward deployed element of the 399th CSH providing Level III medical
and surgical care to over 25,000 Soldiers, eligible civilians, Iraqi
Nationals and detainees. Assessed, planned, implemented and evaluated the
delivery of nursing care in accordance with nationally recognized clinical
practice standards. Mentored and trained 10-12 Officers, 10-15 NCO's and
enlisted soldiers in various medical/surgical procedures and disease
processes. Collaborated with a multidisciplinary team to develop patient
specific nursing and discharge care plans. Mentored patients on various
disease processes and self care needs after discharge. Integral part of

RN Infectious Disease Case Manager
Oversaw day to day medical care for caseload of 30-40 female inmates
diagnosed with HIV and/or Hepatitis C. Collaborated with members of a
multidisciplinary team to coordinate care with medical providers within
correctional institution, outside facility and in a community setting upon
release from facility. Monitored effects of medical treatments including
interactions of HIV, Hepatitis C and routine medications and side effects.
Education of staff and inmates on various aspects of HIV, Hepatitis A,B, &
