WORK EXPERIENCE:
Zachry Construction Co. (September 2007-July 2010) Safety Manager
Covington, Virginia

Duties: I worked as the Safety Manager for the Preventative Maintenance
Contractor, Zachry, at the MeadWestvaco Pulp and Paper Mill in Covington,
VA. I conducted orientation of site specific safety training for all new-
hires; conduct annual refresher training for all employees; conduct
investigation for all injuries, near-miss incidents and accidents, and
complete all required safety documentation. I have composed and implemented
new processes and procedures as needed. I complete order requisitions for
Personal Protective Equipment (PPE) and distribute to all employees as
appropriated by regulation. I have instructed OSHA 10 and 30 hour classes,
as well as instructed American Red Cross First Aid/CPR/AED classes for
employees. I have chaired the Safety Committee for the entire project. I
have overseen the Assured Grounding Program, where all electrical tools and
cords are inspected and color-coded each quarter. I have maintained the
OSHA 300 Log and First Aid Log to ensure their currency and accuracy. I
have conducted safety audits on numerous occasions of the worksite.
Some of the safety related procedures and policies that I have written and
implemented for various issues that have arisen during my work in this
facility are as follows:
I had the opportunity to create and implement the "Reverse Line and
Equipment Procedure" for the reconnecting of piping and equipment due to
the possibility of continued hazards. I conducted an intensive attendance
policy as well as provided training. I composed and implemented a Cell-
Phone Usage Policy for all employees to adhere to while on the job. I
developed a Safety Violation Tracking System for tracking individual
employees' safety infractions in order to determine whether they were
behavior based decisions, or refresher training was required. In order to
create a safer work environment, I implemented the policy of all confined
space entrants to carry an air/gas monitor with them while working inside
of a vessel in addition to the portal attendant having one. The reasoning
for this is that the readings at the portal (entrance) may be fine but the
workers inside could create a pocket of toxic vapors or gases to spread
around them and put them in danger of being overcome or possibly death. I
also implemented the usage of sealable safety eyewear while working in any
dusty environment/. I also implemented the placing of magnetic strips on
the front bill of hard-hats for all employees that use a grinder on any
metal surfaces to prevent airborne metal particles from entering their
eyes. The re-formatting of MeadWestvaco's Confined Space, LEO (Line and
Equipment Opening) and Hot Work Permits is the major accomplishment during
my time on this project.

Turner Universal was the General Contractor for the CRMC (Cookeville
Regional Medical Center) and I was employed by Turner Universal as the Site
Safety Engineer on this project. This project was for the building of a
265,000 square foot addition onto the existing Cookeville Regional Medical
Center. I composed a site specific safety manual and conducted orientation
and safety training for all sub-contractor employees that came to work on
this project. I was in charge of the CCIP (Contractor Controlled Insurance
Program) for all medical claims, incidents and environmental claims that
might arise during this project. I also conducted the pre-employment and
random drug testing as required for all employees. I performed daily safety
audits of the project to maintain compliance with Tennessee OSH Department,
DEQ, EPA, the client's (CRMC) Infectious Control Procedures for the
hospital and also turner Universal's policies.

Safety Engineer of the Owens-Corning FG-2 Rebuild at Fairburn-GA Plant

I was hired as the Safety Engineer for the Construction Manager (Jacobs
Engineering) on for the demolition and complete rebuild on the Owens-
Corning #2 Insulation Line in Fairburn-GA. My major duties were to conduct
drug testing, provide orientation and safety training to all sub-contractor
personnel onsite and conduct safety audits on a daily basis.

I was the Safety Engineer for Turner Construction for the project on
the Virginia Tech
(VT) football stadium in Blacksburg-VA. This project was in excess of
$55 million
dollars. I was in charge of the over-all safety for all sub-contractor
personnel and their
orientation training and any additional training that might be required
for them to
complete their assigned task(s). I was in charge of the CCIP
(Contractor Controlled
Insurance Program) for all medical, incident and environmental claims
that may arise
during the project. I conducted the collection of all pre-employment
and random drug
testing for all site personnel.

SUMMARY:
I have been working in the safety field for over 35 years. I am also the
owner of Alpha Phoenix Company which conducts safety consulting for
different clients as the needs arise. I started this company in 1996 and
consider it a sideline business for now. The services that I offer are as

safety programs and policies, work with their Worker's Comp carrier to
reduce the EMR for the client and work to create a safer work environment

Mr. Bennett has more than 37 years management, operations and safety

both the state and federal levels for safety issues.
