Provides care coordination services to chronically ill patients. Assigned caseload consists of performing ongoing

nursing and psychological assessments, developing and updating child specific care plans, coordinating all aspects of

medical care with the family and providers by making appointments, referrals, assisting family with obtaining

equipment, supplies, medications and providing authorization for CMS funding of needed services when necessary.

Assigned as nurse care coordinator for pilot program, Family Services of Metro Orlando. As nurse for FSMO program,

I coordinate services for children placed in foster care and worked closely with the Department of Children and

Families. Also charge nurse for Hug Me program which coordinates services for children with HIV and leads clinic

duties. Experience in Medicaid and Kidcare systems.
