Opened Medi-Cal Care Coordination Program envelopes of client's signed
documents; retrieved client's birthdays, medical record and medi-cal
numbers from the APS Healthcare C3 program and wrote them on their Release
of Health Information forms. Scanned Consent to Release forms and Medi-Cal
Care Coordination documents to Outlook. Took clients forms from Outlook and
saved them into a file on the computer. I took the saved documents from the
hard drive and attached them to the clients file in the APS Healthcare C3
database; I entered client's information, medical numbers, birthdays, and
date entered into APS Healthcare database in the Excel spreadsheet to be
sent to State Department. I scanned clients Consent to Release information
and Member Questionnaire forms, attached them to client's files in the APS
Healthcare C3 database. I returned duplicate copies of Consent to Release
and Medi-Cal Coordination forms back to the client by mail. I retrieved
client's information on how many times they've been to the E.R. and
hospitalized and input it into a spreadsheet. Alphabetized doctors and
clinics and created binders by the counties they are in.
