Provided support, and coordinated services for the Elderly and Disabled through the EDA

Waiver Program ( Medicaid funded). Engaged clients (upon referral) via telephone for the

purpose of the initial assessment. Conducted a thorough Assessment in person (MDS).

Initiated monthly phone and/ or personal contacts to assess needs and/ or concerns.

Initiated quarterly reviews to monitor and/or observe the quality and effectiveness of
