General Clerk 2: (Horizons Residential Facility)
Provided general clerical support for the office, to include such tasks as data entry/maintenance,
copying/distributing documents and materials, and maintaining record-keeping and filing
systems, etc. Provided clerical support to Supervisor to assist with application review, scheduling
interviews, and documenting the applicants interview answers.
Answered, screened, and directed phone calls to appropriate staff and used Customer Service
skills to assist callers as needed. Opened, sorted, and routed incoming mail and prepared outgoing
mail. Collected information from a variety of sources to prepare needed reports for Supervisor.
Served as Timekeeper for eight staff members, which included entering on a weekly basis, any
time accumulated and used and creating monthly time/leave reports for the Program Manager in a
timely manner. Made weekly work schedule for all staff according to requested leave.
Represented the facility as Safety Committee Representative, which included ensuring all
monthly and yearly safety drills were completed and documented accurately and timely and sent
to the agency Safety Representative. Also attended safety meetings as scheduled.
Entered room and board billing daily on each consumer that resided at Horizons. Responsible for
entering demographic and insurance information into the Carelogic system on new admissions to
Horizons. Verified if consumer had active Medicaid on the Web Portal. Processed and sent
monthly reports to the Consumer Service Board (CSB) showing any admissions and discharges
and other needed information.
Attended, documented, and typed the weekly treatment team meeting minutes and followed
through with any recommendations of the Supervisor for the consumer, which may include
contacting family members for meetings, contacting the agency Nurse for any medical issues, or
making the consumer any physical appointments.
Procurement staff for Horizons, which included ordering all supplies for facility and purchasing
groceries weekly and picking them up. Kept inventory of all supplies in the facility which
included ordering and picking up all medications for each consumer.
Trained new staff hired at Horizons as Instructors, this duty included explaining all policies and
procedures of the agency, admitting and discharging consumers, preparing medication sheets,
how to count the medications when picked up, instructions on how to complete monthly drills,
location of all fire extinguishers and emergency procedures.
Attended CPR/First Aid trainings yearly, as well as ELOT (Essential Learning On-line Training)
