Currently, my job duties include developing and maintaining a secure
working relationship with the medical profession and other allied groups in
order to manage my caseload in a consistent manner. I am responsible for
adhering to policies, procedures, and guidelines rendered by state and
federal regulations. I arrange and coordinate all services for clients with
disabilities (orthopedic or neurological conditions). My job also entails
performing quarterly reviews, monthly contacts, annual recertification,
reading and understanding medical reports, and financial/ statistical case
services reports on a monthly basis. I assist with the training of new
counselors and the interview process of potential new hires. Various other
job duties include: oversight of the attendant care program, supervise
personal care attendants, adhere to guidelines for eligibility
determination for client services. Many of my job duties described in my
previous job carried over to this position with added responsibility.

In my previous position, I was responsible for evaluating the independence
of clients. I developed a plan of care for each client, determined
eligibility based on the client's disability, managed a budget for
Traumatic Brain Injury/ Spinal Cord Injury Waiver, Independent Living
Waiver, Independent Living Grant Program, and Trust Fund program. My job
duties also included assisting clients with maintaining and increasing
their independence. I was responsible for analyzing time logs submitted by
personal care attendants for payments to ensure accuracy. I provided
information to referrals interested in the waiver programs that are
offered.

My job duties/responsibilities included assisting with the provision of
adult day treatment through a Clubhouse model to a DMH approved number of
SMI adults. I maintained consumer confidentiality, completed monthly
summaries as outlined in DMH standards, participated in outreach
activities, made home visits to promote consumer involvement in the
clubhouse. I was in charge of facilitating consumer meetings within the
clubhouse and actively participated in monthly program meetings.

required documentation was completed accurately.
