Lead Case Manager
As Lead Case Manager I was responsible for supporting foster parents in the management and
treatment of foster children and acts as the liaison to all parties on treatment teams. This position
clinically and administratively supervises the foster placement including assessing and locating
appropriate resources needed to accomplish treatment goals. Coordinates and facilitates placement
meetings; may include child’s guardian, community support worker, social worker, foster parents,
and other invested parties; completes paperwork/obtains documents required for placement.
Prepares for and attends court hearings, treatment team meetings, school meetings, and other
community and/or agency reviews that pertain to the child as appropriate. I also maintain current
authorizations for all billable services for assigned caseload; updates/requests reauthorizations as

Social Service Care Manager
As a Social Service Care Manager I was recruited to effectively manage non insured individuals, in
Central Georgia, suffering from Diabetes, Hypertension, Heart Disease, and Depression. As Care
manager I developed proper treatment plans for patients while they are admitted into the program,
as well as keep track of the patient upon their release. I created long term care procedures for each
patient and act as a liaison to patients and families during admission to the program. By listening to
patients, I was able to determine any emotional issues and/or problems and suggest possible
treatments. After meeting program requirements, with care manager’s assistance, individuals are
then assigned a private Physician and a pharmacy. Depending on severity level, members are
contacted on a weekly basis were goals and objectives are discussed.
