Social Worker-Maintaining a caseload of 25-35 patients (Duplin,
Wayne, Sampson, Pender, Lenoir Counties), all using the Medicare
hospice benefit. Social Worker makes 2 home visits per patient each
month using personal transportation to conduct assessments of
patient. Maintaining the responsibility of independently scheduling
patient's visit according to recertification dates. Collaborating
and communicating with Interdisciplinary team of nurses, CNAs,
Chaplain and doctor to provide services that will keep patient's
comfortable and at home until their life expires. Working with food
banks and organization foundation to help provide assistance with
groceries and getting bills paid for low-income families. Providing
families with emotional support. Making visits to doctor's offices
to get paperwork completed for patients. Collaborating with Adult
Protective Services for patients who are in unstable living
