Homeless Prevention and Rapid Re-housing Grant, initiated program in Lyon

County implementing HUD guidelines and creating Lyon county policies and

procedures as well as program process for intake, eligibility and service.

Interview and screen prospective clients, obtain information and required

documentation to determine program eligibility. Inform client of program

requirements, policies and procedures and clearly define what is expected

from the participant.

Contact property manager/owner explain program, negotiate rents,deposits,

late fees and securing all required documentation, assist with locating

rent reasonable housing and performing required HUD inspections as needed.

Oversee participants, assisting with resources, budgeting, addressing

issues that may arise, perform assessments every 90 days for continued

assistance and determining household stability.

Voucher rents and utilities per LCHS procedures, reconcile vouchers to

manual reports, HMIS reports, providing monthly reports as well as HUD

quarterly reports for continued funding.

Emergency services program, Interview and screen prospective clients to
