Complete observations and evaluations of staff on a monthly, initial 90 day, and yearly
bases.
Completing any necessary disciplinary action and/or coaching on Day Treatment LST’s.
Maintaining timesheet; ensuring correct time and millage is put on timesheet and staff
have adequate vacation and sick time to cover hours for absences.
Responsible for preparing and implementing Day Treatment groups that include the
following: cognitive group, anger management, stress management, goal-setting,
communication skills, social skills, vocational skills, memory/study skills, interest
abilities, and values assessments, self-esteem building, exercise, reading
comprehension, etc.
Completing daily curriculum summaries for each core group taught for that day.
Plan weekly outing/activity in the community to implement skills acquired in the group
setting.
Completing Admissions packets on all new day program clients.
Completing and maintaining client information sheets for day treatment clients.
Help create Day Treatment goal sheets for each new client and updating them as the
need of the client changes.
Keeping client information binder up to date with plan of care, waiver assessment, crisis
plan, transition plan, and waiver goals from each discipline.
Filling out incident report for any incident/discoveries that occur between the hours of
8am-4pm. Including notifying guardians, case managers, regulating agencies, and
DCBS if necessary.
Attending monthly IDT meetings to collaborate with the client, families/guardians, case
managers and other disciplines to form a treatment plan specific to that client.
Maintain monthly progress notes on all clients
Ensuring carry over from other disciplines into the Day Treatment setting.
Ensuring that the program model is followed, that all clients are keep in a therapeutic
environment, and state and company regulations are followed by all staff.

1. Resident Supervision: Supervise residents at all times to ensure safety and
compliance with program expectations. This includes daily chores, hygiene, completion
of IL modules, and activities.

residents on daily point acquisition.

approval transport residents to appointments.
4. Provide Behavioral Management: Including praise, natural consequences, education,
de-escalation and Safe Crisis Management as necessary for out-of-control clients.
5. Documentation: Complete all necessary daily, weekly, and monthly documentation.
6. Meetings & Training: Attend and provide input to team meetings, and monthly
trainings. Maintain A certificate in SCM. Complete required 40 hours of training annually.
Participate in campus QI through attendance and input.
7. Facility: Maintain cleanliness, organization, and functioning of cottage areas, office,
and equipment.
ADDITIONAL INFORMATION
1. Conduct group session with residents as outlined by Program Manager.
2. Assure residents are on time to appointments and for medication administration.
