Reid Hospital, Richmond Indiana, Emergency room reports, history and physicals, discharge summaries, and psychiatric reports. Central Indiana Orthopedics of Muncie Indiana, and Anderson, Indiana. Clinic notes, consultation notes, cover letters, workman’s compensation notes, x-ray reports, and orthopedic surgeries. Former account was Community Hospitals East, and North of Indianapolis, Indiana responsible for emergency room reports, history and physicals, discharge summaries, cancer care reports, and psychiatric reports.
February 2009 to August 2009 MedTech Resources. Acute care medical transcriptionist on the University of Kentucky Medical Center at Lexington. Responsible for typing medical documentation for emergency room and other clinics.
March 2005 to November 2005 and
December 2006 to February 2009. TransMed Systems Inc. Medical transcriptionist on the Flagstaff Medical Center account. Duties include transcription of acute care, emergency room reports, and other documentation from all departments with the exception of Radiology.
