OBJECTIVES
To obtain a job which would provide me the opportunity to apply acquired
knowledge in the field of social work, as well as continue developing my
skills in interpersonal relations. To obtain additional knowledge, and
gain experience, in areas related to human conduct, such as public
relations, interviewing the public, interpersonal problems analysis and
solutions, and others, including jobs requiring the use of technology. To
have an opportunity to use my skills in a work environment that would
prepare me to hold positions of responsibility in those fields.

EXPERIENCE
Case Manager June 2006 - June 2012
AIDS Help, Inc. Key West, FL
As a case manager I had sixty clients assigned to me. I provided assistance
in applying for SSDI, SSI as well as appealing every denial case. As a PAC
Medicaid Provider I provided assistance to apply for Medicaid, PAC Medicaid
and Food Stamps benefits as well as follow up on their cases. I provided
assistance for disabled clients to have access to Medicare. I also filled
out applications for I - 94, Employment Authorization, Permanent Resident
Card, filled out Fee Waiver Requests and follow up cases related to
immigration status. I coordinated services related to housing on SHOPWA,
Section 8, public housing and affordable housing. I assisted clients in
creating a resume as well as search for jobs. I provided assistance in
unemployment benefits application and appeal for denial cases, created
Service Authorizations for low income clients in order for them to have
access to food vouchers. Located food pantry for clients in need of food
but ineligible for food vouchers or food stamps benefits. I monitored HIV
counts, interpreted lab results, assisted with the clients to their medical
and psychological appointments as well as SSDI, SSI appointments and
provided translation services as needed. I did initial intake, 6 months
assessment, Plans of Care and home visits as required.

As a social worker I had to provide orientation to participants regarding
the Domestic Violence Law of Puerto Rico (Law 54). The participants were
battered women or women that had suffered any type of violence (physical,
emotional or psychological). I assisted participants to complete the
request for a restriction order against the aggressor and make crisis
interventions, as necessary. I had to explain to the victim of domestic
violence the general effects and scope of any restriction order issued by a
judge against the offender. I interviewed the victims to ascertain any
needed guidance. I went to the court with the victims and provided any
necessary assistance, especially providing to the judge any information
given to me at the office of the program, when they were too nervous or
emotionally affected to talk on their own behalf. In high-risk situations,
I took any necessary steps to provide secure shelter to the victim,
including transportation. As needed, I prepared the referral documentation
for the police to take the person to the shelter. I referred participants
for psychological services and provided orientation and referral services
for housing, when needed.

As a Social Worker I performed short and emergency interventions, follow-
up appointments, crisis interventions, escort and support during the
judicial process for battered women. I also provided vocational guidance
and helped them during the admission process to universities and colleges
in the area. I established coordination with agencies and service
organizations for our clientele, as well as educational and recreational
activities for them.

I worked performing interventions with individuals and families through
telephone or face to face contact, coordinating educational fairs,
relocating candidates for undergraduate studies in different educational
institutions and interventions with individuals engaged in prostitution,
drug and alcohol abuse and with homeless persons. I had to prepare social
profiles, conduct initial interviews for services, as well as coordinate
contacts between claimants and agencies providing services. I also
conducted studies on educational needs for students at the Sor Isolina
Ferr Center, sessions with school dropout students to motivate them to
take preparation courses for the 9th and 12th grades equivalence exam.

I obtained my degree, Cum Laude, in May 2002. There was an interruption in
studying at the University when I got married and went to live in the
United States from about 1996 to 2000. I was able to return to Puerto Rico
to complete my degree. I conducted my social work practice at the Centro
Sor Isolina Ferr in Ward Caimito of San Juan, PR. My elective credits are
in education and the major is in social work. I studied at the Arecibo and
the Metropolitan (Cupey) campuses of the Interamerican University.
