Responsibilities: Admissions Nurse-Responsible for assisting in the
determination of patient appropriateness for hospice care, assisting
patient and family with end of life goal setting, admission into the
program/services the patient is eligible for and developing a care plan in
conjunction with patient goals, medical team and Four Season's policies.
Provide crisis care interventions when needed during admission process.
Achievements: Co-Created patient's End of Life care through the admissions
