Caretel Inns of Linden, Skilled Nursing Facility.
Responsibilities included initial evaluation of patient, utilized Berg
Balance Scale for standardized testing/documentation of progress,
development of short and long term goals, utilization of Giftwrap
computer software to complete detailed weekly progress notes, analysis
and completion of recertification of patients, home safety
evaluations, development and instruction of treatments including
functional activities and rote exercise, wheelchair modifications, use
of multiple modalities to increase effectiveness of treatments,
providing self-care training, as well as development and instruction
in functional adaptations, use of adaptive equipment and compensatory
strategies in order to increase independence.

instruction to patients in use of adaptive equipment. Population
served included individuals receiving rehabilitation services
secondary to stroke, orthopedic surgery, chemotherapy, prolonged stays
in the ICU and cardiac disease/surgery in order to increase functional
independence of patient.
Concentrated on clients who underwent placement of Ventricular
Assistive Devices including the XVE and HeartMate II. This included
education on the operation, care, and specific alarms of the device,
as well as reintegration into the community and coping with the device
in everyday life.
