EMPLOYMENT
GRANDVIEW HEALTHCARE CENTER – NOVEMBER 2010 TO JUNE 2013 Director of Nursing for 180 bed unit which accommodates long term care, assisted living and acute rehab patients. Responsible for the management of the nursing department in relation to direct care of patients. All other duties as appropriate in relation to operations of the building and educational needs of patients and staff.
SUNGROVE VILLAGE CARE CENTER-JULY 2009 TO OCTOBER 2010 Assistant Director of Nursing for 128 bed unit which accommodates geriatric long term care and acute rehab patients. Duties include Infection Control, QA, ongoing education to staff, oversee wound program, Incidents (QA/QI), back up MDS coordinator (recently trained on new MDS 3.0) and to assist the Director of Nursing as appropriate.
MERCY CARE/SHALLER ANDERSON/AETNA – APRIL 2007 TO JANUARY 2009 RN Home Health and SNF Case Manager for ALTCS LTC staff of RN’s who facilitate care to medically complex ALTCS members that reside in skilled nursing facilities, home and community based and assisted living communities. The RN ALTC LTC team of nurses which case managed and supervised members that were ventilator patients in skilled nursing facilities and home and community based, as well as the medically complex. Overall continuing education of non-clinical case managers and new initiative processes to ensure continued excellent care of MCP members. Initiated Hospice Program with IDT meetings to review and discuss medically complex members that would be Hospice appropriate. Also for UM, reviewed and analyzed “frequent flyers” on hospital admits and to manage and/or transfer these specific members under the care of a registered nurse for cost effectiveness which was part of the Utilization Review process.
IMMANUEL CAMPUS of CARE – MAY 2006 TO MARCH 2007 Director of Nursing directly responsible for the management of Nursing Department staff for direct patient care of 216 bed facility. The building provided care for the geriatric population in long term care, two secured dementia units and one 30 bed rehab unit and assisted living campus. Facilitated CQI monthly meetings, reporting of QI results to corporate. Initiated bowel and bladder program, park and walk, wound program and wound nurse. Initiated and secured RN instructor for approved C.N.A. training program approved by the ASBN. Oversee staff recruitment and reduction of registry.
CHRIS RIDGE VILLAGE – JUNE 2005 TO MAY 2006 Director of Nursing in a 79 bed facility; 20 beds were for sub acute and the remainder were long term care. Directly responsible for the management of nursing staff for direct patient care. Initiated wound program with wound nurse, bowel and bladder program. All other routine DON duties as appropriate.
INFINIA of SCOTTSDALE – JANUARY 2004 TO JUNE 2005 Director of Nursing 130 bed facility with acute rehab, long term care and secured dementia units that housed private pay, managed care and ALTCS residents. Management of Nursing Department for direct patient care, initiated RNA program, Park and Walk and Falling Star Program. Attended monthly CQI meetings. Oversee staff recruitment and reduction of registry.
GLENCROFT CARE CENTER – JUNE 2002 TO JANUARY 2004 Assistant Director of Nursing in a 225 bed facility responsible for 3 out of 6 units. Monitoring floors for accuracy in charting, audits for MAR/TAR’s, pain control program and facilitating weekly focus meetings on improving the environment for the residents and staff. Attending all Department Head meetings, QA, weekly nursing meetings and to assist Director of Nursing as appropriate.
BRIGHTON GARDENS – NOVEMBER 2001 TO JUNE 2002 Director of Nursing in a 52 bed sub acute/long term care private pay units, in addition, a secured Dementia unit and assisted living unit. Responsible for management of nursing staff in direct patient care, ongoing recruitment, staffing, implementation of protocols in policy and procedure manuals and facilitating monthly reports to corporate for CQI.
HOSPICE OF ARIZONA – JUNE 1999 TO NOVEMBER 2001 Patient Care Manager for West team of RN’s, LPN’s and C.N.A.’s. Professionally managed LPN’s and managed the direct care given to patients requiring Hospice services in skilled nursing facilities as well as home and community based patients. Visited assisted living homes with the Marketing Department to educate on the clinical aspects of hospice services. Also did outreach for ongoing education to skilled nursing facilities on hospice philosophy, pain control and end of life issues
DIGNITA HOSPICE – MARCH 1998 TO JUNE 1999 Started as a PRN field RN and transitioned to full time position as the RN Manager for the West Team. Managed a team of RN’s and C.N.A.’s that did home and skilled nursing facility visits for Hospice patients. Assisted with weekend on call triage, new admissions and occasional home and skilled nursing facility visits.
CIGNA HEALTH PLAN – NOVEMBER 1985 TO MARCH 1999 During the 14 year period, entered as a brand new LPN working back office in Family Practice. Cross-trained in all other departments; OB/GYN, Allergy, Dermatology, Internal Medicine, Podiatry, and Urgent Care. Advanced to team leader in OB/GYN department just prior to admission to Mesa Community College for RN program. Out for two semesters and returned as a brand new RN in the triage department for Family Practice and Pediatrics. Worked Urgent Care as an RN, advanced to Supervisor at the SCW facility managing the team of physicians and nurses who cared for CIGNA’s HMO geriatric patients. In 1999 worked on a prn basis with CIGNA’s hospice home program.
BOSWELL MEMORIAL HOSPITAL – OCTOBER 1993 TO JUNE 1994 Staff RN for Urology, GYN, and Surgical floor. Staff RN with total patient care, ongoing education for patient and families to ensure a safe discharge to home with the proper knowledge of their own personal outcomes.
