assigned to the case worker for monitoring. Home visits were completed at least quarterly. Follow up
with the Young Adult Program contracts were conducted. Renewed contracts with clients in the Young
Adult Program every six months. Attended all court hearings involving out of home cases. Notified the
District Supervisor of unusual court orders. Informed the District Supervisor of findings of No
Reasonable Efforts.
