Breast Care Advocate
Part of a newly established breast cancer advocacy program started in Fall 2011 initially working
with the Jewish Premier Gynecology & Obstetrics group. One of two registered nurses on a
team responsible for all women diagnosed with breast cancer within the JHSMH system. Duties
include connecting with patients immediately after diagnosis, making referrals to surgeons,
oncologists, community resources. Coordinated care between patient, primary care physician,
surgeon, oncologist and many others working as a team to facilitate the best outcome for the
patient throughout her treatment plan using resources within the JHSMH system. Developed
unique care plan for each individual focused on their needs while providing timely, high quality
treatment and minimizing gaps/delays in care. Worked with community resources for cancer
patients from the American Cancer Society, Susan G. Komen, Gilda’s Club, Livestrong at the

Nurse Navigator
Served as navigator in a pilot position with Norton Healthcare. One of only five RN positions
created and placed throughout Physician Services that focused on patients with certain
chronic diseases to help close the gap between hospital stays and primary care provider
and lower 30 day readmission rates. Duties included tracking and connecting with
hospitalized patients and their family members to then follow up after their stay and help
coordinate care post discharge. Focused on CMS regulations and guidelines for core
measures to increase patient outcomes through education and assistance with improving
self-care measures at home after a hospital stay. Worked closely with home health
agencies and hospital care managers to increase quality of care through the continuum.
Focused on increasing education and compliance for preventative care screenings on all
adult patients regardless of health status in the physician's office by developing a
