Retirement Community are long term care, acute care, and assisted living facilities.

To insure meeting medical, ancillary, and psychosocial needs closely managing a substantive case load of

acute care and long term care clients requires attention to details.

Providing strong emotional support during the admission process assisting in Single Entry Point programs

evaluation, outreach, and resource development within regulation and guidelines established by state

agencies. Assisting in financial applications to the state of Colorado Medicaid (ULTC100.2), Child Health

Plan Plus (CHP), Cover Colorado, Denver Health Medicaid, and other insurances as needed contributed

to the core team to foster a communal environment conducive for physical and emotional healing.

Acting as an information and resource guide for clients, family members, and other professionals

while coordinating ancillary services for vision, dental, and audiology needs, frequently resulting in

financial support requests to the state of Colorado via the PETI (prior eligibility for treatment of income)

system of which close attention to details, monitoring, and follow up is critical for successful outcomes.

Managed psychosocial aspects of facility sponsored psychopharmacology committee. Successful tracking

of psychotropic medication(s) efficacy, state required dose reduction(s), appropriate diagnosis, behavior

changes, and other required documents, as needed, to maintain compliance with state and federal

governments.

Process improvement committee member required monthly quality improvement reports related to client

complaints/complements, psychotropic medication use, rate of reduction(s), classification, diagnosis,

referrals for ancillary services, Medicaid issues, advance directives, and plan of correction, if needed.

Timely completion of state and federal required Minimum Data Set (MDS 3.0) on all facility clients upon

admission, five (5), fourteen (14), thirty (30), sixty (60), and ninety (90) day intervals requires

understanding of clinical needs, analytic problem solving, documentation, and care planning strategies.

Professional relationships established with home health agencies, providers of durable medical

equipment, ancillary services providers, oxygen needs, hospice, and mental health services providers, as

well as translation services for non-English speaking clients and family members.

Managed “Family Counsel” through implementation of psychosocial support systems acting as a liaison

for clients and families for community programs, advance directives “Colorado Medical Orders for Scope

of Treatment” (MOST), provided introduction to skilled rehabilitation services question and answer

sessions as needed for client and family comfort and well-being.

Managed a reliable source of translation and recreation volunteers to insure meeting the communication

needs of non-English speakers as well as recreation needs/desires for client(s) emotional and physical

well-being.

Utilized a focus group comprised of clients and family members to initiate a facility smoking policy and

procedures pamphlet.
